Covid-19 Parental Consent Form

Emma Louise Dance Club agrees to adhere to the UK government and the World Health Organisation (WHO) recommended ways to prevent the spread of infection.

Emma Louise Dance Club ensures the appropriate measures are in place; through displaying posters promoting handwashing and ensuring hand sanitiser/and or soap is available on the premises.

If your child (or anyone in your household) is unwell, even with a mild cough or fever, they should stay at home and will not be permitted to enter onto premises where Emma Louise Dance Club classes are held.

The parent/carer agrees that Emma Louise Dance Club is not responsible for any spread of infection or contraction of Covid-19.
